My wife thought her migraines were gone until she developed vertigo and ear related symptoms

My 30yo wife started having chronic migraines a few years ago, no issues with vision or sound or anything, just a bad headache, no known triggers (though her work is very stressful and she’s actively trying to combat that now). Probably occuring 5 days a week. She was eventually put on a monthly Agovy injection which completely stopped them in their tracks for good for 2 years. We decided we wanted to try and have a child this year, so she came off Agovy 6 months ago. She was deathly afraid of this, so she started going to a naturopath at this time. With manual adjustments and supplements her migraines never came back while off Ajovy, for the last 6 months essentially. Thank god.

This last Thanksgiving during a yoga class, she was hit with intense vertigo (during an inversion) and ear pain that were both on and off for several days. She went to several doctors - emergency room, urgent care, GP, ENT, dentist- and they all pretty much said it was probably fluid in her ears. During these few weeks, she was prescribed Sudafed, muscle relaxers, and steroids among other things. The Sudafed and steroids seemed to help only while she was on it. While the vertigo ultimately went away, the ear pain on one side was relentless after she stopped the medications.

Per the recommendation from her dentist, she saw an Oral and Maxillofacial surgeon. They did an xray and ruled out TMJ. Knowing her migraine history, they concluded confidently that these were completely new and different symptoms from migraines again. While a seemingly definitive answer was nice, it was a hopeless answer especially since migraines seem very tricky to solve. So, she was prescribed Qulipta last week. They also suggested jaw exercises and a mouthguard which she’s getting next month.

During the week of taking Qulipta, her ear pain has mostly reduced, and she’s even had days where it was completely gone, but also some days where it’s back to normal and full pain. To add to this, she was hit with similar intense bouts of vertigo last night and this morning for the first time since this all started. This was extremely frustrating because she thought that was resolved and we’re unsure if that is a migraine symptom.

Has anyone had a similar story to this, or any experience with vertigo like this? It just seems like the vertigo is a part of this puzzle that doesn’t make sense, especially with how it was originally caused by the inversion in yoga. Also, the fact that the steroids and Sudafed seemed to work is confusing because I wouldn’t think they’d help migraines. Any insight would be greatly appreciated. Thank you!
