Enrolling at MAPUA this year

I am a grade 12 student just trying my luck and enroll to MAPUA University as a freshmen. I'm having a bit of simple trouble in the admission page. What should I place in the blanks: Grade 11 Science grade, Grade 12 Math grade and Grade 11 English grade when we had 2 different subjects in the first and second semester (ex. In math: 1st Sem - Gen Math, 2nd Sem - Statistics and Probability; In english: 1st Sem - Oral Communication, 2nd Sem - Reading and Writing) in Grade 11? Should I put the average of the 1st and 2nd Sem grades I had in those subjects (English, Math, and Science)? I'm really lost right now, thanks for y'alls advice.
